Systems seem to randomly fail, and as soon as they are fixed, a new one falls over. Even worse, when it comes time to "fix it right" the team gets pushback from the business leaders, who want more features. After all, our startup is on a fixed budget. The second biggest form of waste is fixing bugs in software nobody wants.
